Overview

Chronic Care Staffing is seeking a compassionate and detail-oriented Remote RPM Coordinator – CMA to support patients living with chronic or high-risk conditions through Remote Patient Monitoring (RPM). In this role, you will monitor biometric data, educate patients on device usage, escalate critical findings, and act as a crucial liaison between patients and their healthcare providers. Your work helps improve patient outcomes, promote adherence to care plans, and reduce hospitalizations through real-time monitoring and timely intervention — all from a fully remote, HIPAA-compliant home office.

Responsibilities

  • Conduct patient onboarding and device education calls for new RPM enrollees.
  • Monitor daily biometric data (blood pressure, glucose, weight, oxygen saturation, etc.).
  • Identify abnormal readings and follow escalation protocols to notify providers.
  • Perform monthly RPM engagement calls and address patient concerns.
  • Troubleshoot device connectivity or usage issues.
  • Communicate with patients and caregivers about their conditions, goals, and progress.
  • Support patient self-management and adherence to provider-developed care plans.
  • Coordinate with healthcare teams to ensure timely responses to alerts.
  • Document all interactions and interventions accurately in the EHR/RPM portal.
  • Educate patients on lifestyle modifications, risk reduction, and monitoring importance.
  • Comply with CMS RPM billing and documentation requirements.
  • Meet productivity expectations for monitoring and outreach.
  • Represent Chronic Care Staffing professionally and compassionately at all times.

Requirements

  • Unencumbered active CMA or RMA certification.
  • Active BLS certification.
  • Ability to communicate clearly and compassionately via phone or messaging.
  • Excellent documentation, time management, and critical thinking skills.
  • Experience with EHR systems and cloud-based platforms.
  • Familiarity with Google Suite.

Nice to Have (Optional)

  • Strong understanding of Remote Patient Monitoring protocols and platforms.
  • Proficiency in recognizing clinical red flags in biometric data.

Home Office Requirements

  • A HIPAA-compliant workspace free from distractions.
  • A room with a lockable door to prevent accidental PHI exposure.
  • High-speed internet connection.
  • Use of a CCS-approved computer with dual monitors.
