Reason for RPM Alert Call:

Critical BP alert received through the RPM platform. Patient’s current BP reading is 153/105, which is above the patient’s assigned safe range of 80–140 systolic / 55–90 diastolic. Follow-up call required to assess symptoms and determine need for escalation.

Recent History:

• Recently diagnosed with acute sinus infection.

• Completed Zithromax 3 days ago.

• Currently taking an over-the-counter decongestant for cold/sinus symptoms.

RPM Data:

• Current BP: 153/105

• Trend: Readings over the last several days have been borderline but within range until today.

• No missed data uploads.

Symptoms Reported:

• Shortness of breath

• Racing heart / palpitations

• Denies chest pain, severe headache, vision changes, confusion, neurological symptoms, or weakness.
