Description

As a Care Management Coordinator at Pediatric Home Service, you will play a key role in ensuring that our young patients receive the best care possible in the comfort of their homes. Your daily responsibilities will include coordinating care plans, connecting families with community resources, and serving as a vital communication link between families and healthcare providers. You'll engage with families, empowering them to navigate the healthcare system and addressing their concerns while ensuring our patients' needs are met efficiently. A typical day may involve scheduling appointments, processing paperwork, and collaborating with an interdisciplinary team to support pediatric patients with complex medical needs.

Requirements

• 1–2 years of relevant experience in care coordination, home health, customer service, or pediatric healthcare settings.
• Experience working with vulnerable or medically complex populations is strongly preferred.

Nice to Have

• Knowledge of community-based services and insurance processes.
• Familiarity with HIPAA compliance and medical terminology.
• Background in social work, advocacy, or case management support roles.
• Experience in homecare, infusion services, or complex pediatric care.
• Familiarity with pediatric advocacy groups and disability services.
• Comfort working in a fast-paced, mission-driven healthcare environment.
• Bilingual (Spanish/English) skills are a plus but not required.
